The Consortium partners with CHCN to coordinate and implement programs and policies that ensure high-quality health care to patients and decrease racial, ethnic, and economic health disparities. While the Consortium and CHCN focus on different areas, we have seven of eight member clinics in common and are committed to work in partnership to ensure the viability of our member clinics and the continued provision of high quality health care.
